• Specifically, how am I likely to die?
• What are the best outcomes that I might reasonably hope for?
• What are the worst outcomes that I might reasonably have to deal with?
• Thinking about the more serious or disruptive problems that my medical and personal situation might cause, what can be done to plan ahead to manage them?
• How will this illness and its treatment affect me and my family?
