David Cardenas, Director Transitions offers one-on-one support for people and their caregivers through the dying process, particularly in home settings. We seek to make the death process more life affirming and positive. We can deliver services in hospices or home settings. We do not provide health care services. We work with all types of terminal illness, including AIDS and cancer. We also offer training workshops for caregivers of people who are dying; persons who are dying may attend if they wish. The workshops cover practical, emotional, and spiritual issues in dying. Topics include what to expect during the dying process, basic comfort care, how to arrange for hospice care in the home, nutritional issues, bereavement referrals, taking care of yourself as a caregiver, and practical aspects of dying such as planning a funeral. The topic of self-deliverance often comes up, and we approach that issue in a non-judgemental and confidential manner. Transitions is a 501c3 non-profit organization. There are no fees for our services, but contributions are encouraged. We are bilingual in Spanish and English.
